The Iowa State University Cheerleading tryout is a 3-day process that is held in person on the following dates with potential cuts made throughout the weekend.

Athletic Exam Required for participation in the tryout.

An athletic examination form will need to be prepared and signed by the participant's healthcare provider prior to the start of the tryout. A copy of Iowa State Spirit Squad's athletic exam form can be downloaded here (PDF).

Participation Agreement and Waiver Review the following. You must agree in order to complete the registration.

Athletics Department – Spirit Squad Tryout Registration

I am aware of the substantial risks involved with stunting, gymnastics, cheerleading, dancing, tumbling, and performing in general for all Spirit Squads. I am a healthy person, knowledgeable of the athletic abilities required to become a member of the Iowa State Spirit Squad, and prepared to participate in the tryouts for the Iowa State Spirit Squad. I will take all of the necessary precautions before participating in the Iowa State Spirit Squad Tryouts and during the season at Iowa State. If an injury should occur, I will take full responsibilities for any medical supervision or care that may be necessary (via Athletics Training procedures & protocol per my Spirit Squad Coach).

I herebyrelease, waive, discharge and covenant no to sue the State of Iowa, the Board of Regents (State of Iowa), Iowa State University, the Athletic Department of Iowa State University, and any of the officers, servants, agents, and employees of the above-mentioned entities (hereafter referred to as RELEASEES) for any liability, claim and/or cause of action arising out of or related to any loss, damage or injury, including death, that occurs as a result of my participation in any activities in conjunction with the Iowa State Spirit Squad Tryouts and during the season if I am chosen to the Spirit Squad. I agree to INDEMNIFY AND HOLD HARMLESS the RELEASEES whether injury is caused by my negligence, the negligence of the RELEASEES or the negligence of any third party. I further agree that this Release and Waiver Liability shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personnel representatives, if I am deceased, and shall be deemed as a RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE the above-named RELEASEES.

I have the read the above information in its entirety. I understand that the Iowa State University Athletics or the ISU Spirit Squad is not liable for any injuries sustained in conjunction with an event. I certify that the information I have provided in this application and accompanying documents is true and correct to the best of my knowledge.

THEREFORE, I hereby voluntarily give my consent to participating in the Iowa State University Spirit Squads.

I am over 18 years of age, or the legal parent or guardian for the above participant. I have read and understand all of the conditions set forth above, agree to those conditions, and do so voluntarily. You must agree to the Participation Agreement and Waiver.
